Fondaparinux: The First Pentasaccharide Anticoagulant
نویسنده
چکیده
Despite advances in anticoagulation therapy, venous thromboembolism (VTE) remains a major clinical concern. The incidence and risk of VTE following major orthopedic surgeries is well documented.1 In addition, because of the clinically silent nature of deep vein thrombosis (DVT) and pulmonary embolism (PE), and the associated morbidity and mortality, a clear rationale for VTE prophylaxis following orthopedic surgeries has been demonstrated.1,2 Fondaparinux is the first in a new class of pentasaccharide anticoagulant agents, and is FDA-approved for use in the prophylaxis of DVT that could lead to PE in patients undergoing hip fracture surgery, hip replacement surgery, and knee replacement surgery.3

Extended Physicochemical Characterization of the Synthetic Anticoagulant Pentasaccharide Fondaparinux Sodium by Quantitative NMR and Single Crystal X-ray Analysis.
Fondaparinux sodium is a synthetic pentasaccharide representing the high affinity antithrombin III binding site in heparin. It is the active pharmaceutical ingredient of the anticoagulant drug Arixtra®. The single crystal X-ray structure of Fondaparinux sodium is reported, unequivocally confirming both structure and absolute configuration. The iduronic acid adopts a somewhat distorted chair con...
متن کاملAbility of recombinant factor VIIa to reverse the anticoagulant effect of the pentasaccharide fondaparinux in healthy volunteers.
BACKGROUND The novel anticoagulant fondaparinux proved to be effective and safe in the postoperative prevention of venous thrombosis. Current phase III trials with this synthetic selective factor Xa inhibitor focus on its use in the treatment of patients with venous and arterial thrombosis. As with any anticoagulant therapy, there is a risk of bleeding complications; hence, a strategy to revers...
متن کاملAnticoagulant-related skin reactions.
Cutaneous reactions have been reported during anticoagulant therapy with coumarin derivatives, unfractionated and low molecular weight heparins, heparinoids, danaparoid and hirudins. Anticoagulant-induced skin reactions vary from local allergic manifestations to skin necrosis. In patients with allergic reactions, diagnosis and crossreactions between anticoagulants can be confirmed by intracutan...
متن کاملLepirudin in the management of patients with heparin-induced thrombocytopenia
Heparin-induced thrombocytopenia is a strong risk factor for the development of arterial and venous thromboembolic events. In patients clinically suspected for HIT, immediate cessation of heparin treatment and continuation of anticoagulant treatment using alternative anticoagulants is mandatory in order to minimize the risk of thrombotic events. Alternative anticoagulants that have been success...

The elucidation of the structure of the antithrombin binding sequence in heparin has given a large impulse to the rational design of heparin related drugs. De novo chemical synthesis of the corresponding pentasaccharide as well as simplified analogues has provided very specific, antithrombin-mediated inhibitors of factor Xa with various pharmacokinetic profiles. Fondaparinux and idraparinux are...
